The barcode and ISBN cluster covers failures where encoded identifiers, quiet zones, metadata, and back-cover placement fall out of sync. These issues matter because barcode logic is both visual and machine-readable.
A barcode may look acceptable to a human reviewer but still fail if the quiet zone is violated or the encoded ISBN does not match the title metadata.
The most common causes are reused barcode assets after title changes, cover templates shifting the white box position, back-cover redesigns that invade quiet zones, and metadata mismatches between dashboard settings and the cover itself.
These failures become more common late in production when marketing edits happen after the original barcode placement was approved.
Confirm the final ISBN first, then verify that the generated barcode, metadata, and back-cover layout all reference the same identifier. After that, inspect quiet zone spacing and cover template alignment.
